Practice Nurse

Full time 37.5 hours per week (part time may be considered, minimum of 30 hours per week)

Salary – GBP28,275 – GBP32,175 (pro rata for part time position)

An exciting opportunity has arisen for an experienced Practice Nurse to work for Oakleaf Care; an inpatient brain injury rehabilitation service for up to 116 male residents.

Job responsibilities include:

?? Monitoring and management of chronic disease areas such as Asthma, Hypertension, diabetes.

?? Conducting investigations, ECG’s

?? Adult vaccinations

?? Dressings and wound care

?? Requesting pathology tests, for example urine cultures, swabs

?? Following agreed clinical protocols and liaison with the Oakleaf on site Dr or GP surgery as necessary

?? Venepuncture

?? Running and recording monthly health screening results

?? Assist the nurses with physical health care planning

?? Assist with new admissions’ physical health care screening

?? Any other delegated duties appropriate to the post

Skills and knowledge

The applicant must have

?? Valid NMC registration

?? A demonstrable passion for caring and a flair for helping people maintain their independence and choice in homely surroundings

?? A genuine commitment to adhering to the NMC Codes of Conduct and expanding your professional development.

?? A respectful, courteous and compassionate nature and belief in giving your personal best at all times.

?? Experience of care planning and report writing

?? Good communication and teamwork skills, ability to work without direct supervision and determine own workload priorities.

Experience

?? Experience of working as a practice nurse or community nurse

?? Experience of chronic disease management

?? Good interpersonal skills and a willingness to work flexibly as part of a team.

?? Able to work under pressure and have clear systems/ strategies in order to prioritise workload
